Planning for Your Appointment
At Springboard, we strive to make your experience as comfortable and stress-free as possible. Here are some suggestions on how you can best prepare for your appointment.

What do I need to bring or submit for an assessment appointment?
- Your completed questionnaires
- Referral from your doctor (if applicable)
- Valid Ontario health card
- Any previous assessment reports (including psychological, speech therapy, medical or school assessments, and IEP/IRCP)
For kids, teens and students:
- Please also bring or submit any questionnaires completed by your teacher, and report cards.
For online appointments:
- You’ll need a private space, headphones – and a computer, of course. We also strongly encourage snacks, paper, a pen or a pencil (plus toys and other drawing tools for kids) – anything that makes you feel more comfortable!
What do I need for my Straight to Coaching initial appointment?
For an online appointment you’ll need a private space, headphones and a computer, of course – plus anything that makes you feel more comfortable!
Please also bring:
- Your booking confirmation
- Your completed questionnaires
- Informed consent forms
And most importantly, bring yourself. We’re interested in learning more about where you’re at today and any goals you have for the future. We’ll talk about your understanding of ADHD, your strengths and interests, and your priorities for change, as well as your coaching and psychotherapeutic needs.
At the end of the session, you’ll get a summary what we discussed, outlining your priorities and suggesting a few activities to get you started.
What do I need for my coaching and therapy appointment?
For an online appointment you’ll need a private space, headphones, maybe a pen and notebook and a computer, of course – plus anything that makes you feel more comfortable!
If you wish, you can prepare by thinking about areas where you’re having difficulty, or strategies you’d like to try. But all you really need to bring to the session is yourself. Your coach will meet you where you are in your ADHD journey and work alongside you toward whatever goals you define together.
I have an in-person appointment. How do I get to Springboard?
Public transit: We’re conveniently located one block north of Rosedale subway station on TTC Line 1 (Yonge-University).
Driving: Parking is available on the streets surrounding Springboard Clinic. There are also two Green P parking lots nearby:
- North of Springboard at 15 Price Street
- South of Springboard at 1010 Yonge Street
